My policy has been lost or stolen. How can I replace it?

The first thing you need to do is contact the company you have the policy with. Each company may have slightly different rules for the replacement of a policy, but at a minimum you should expect to have to provide accurate personal information in order to have it replaced. You may have to supply your social security number, if you provided it originally.
